O'Brien 95th Air Base Wing Public Affairs EDWARDS AIR FORCE BASE, Calif. -- The new Near-Miss Form, available on Centernet allows base personnel to advise the Air Force Flight Test Center Safety Office of potential risks on base. "This (form) lets us fix things before people get hurt," said John Kalita, AFFTC ground safety specialist "We get out and identify as many as we can but nobody is more aware of a risk than the people who come across them every day. Mr. Kalita encourages everyone to fill out the form for anything they deem a potential risk. "Use this form. That's why we have it," he said. "The form is easy to fill out and we look into every one we receive. If you're not sure if it's a risk, let us know. We would rather go investigate a non-risk than have someone leave a risk unidentified and leave everyone susceptible to it." In circulation for about a month now, Mr. Kalita said primarily, people have used the form to identify risks on the road. The form isn't limited to only traffic safety, and the Safety Office encourages everyone to identify risk elsewhere as well. Any near miss or anything one sees as a problem, they can let the Safety Office know by filling out the form. Once they receive it, they will evaluate the problem and if it is an actual hazard, they have the authority to require it to be address immediately. "If something seems wrong, out of place or unsafe, fill out the form and we'll evaluate it," he said. When the safety office receives the form, they visit the site of the potential hazard to evaluate it. "First we identify if it is actually a risk, then we take it to the proper authority to get it fixed. Risks identified as a hazard by us are considered priority and fixed quickly." "This is in place to keep you safe," he said. "If you take action, we can take action and together we can make Edwards safer."
