Wind damage claim information Published March 8, 2011 By Air Force Flight Test Center Judge Advocate EDWARDS AIR FORCE BASE, Calif. -- If you suffered damage as a result of Monday's wind storm, you may be able to file a claim for that damage with the Air Force. Department of Defense Personnel (active duty and civilians) must file their claim online at claims.jag.af.mil. All others must use form SF-95. When you file your claim, you should have the following supporting documentation: 1. Orders and any amendments assigning you to the place of incident (i.e., Edwards AFB). 2. Copy of storage contract (if stored on a lot) 3. Repair estimate(s) 4. Witness statement by supervisor, family member, friend or co-worker establishing the location of your vehicle at the time of the incident 5. Copy of vehicle title and registration 6. Photos of damage and of your property before the damage, if you have them 7. An Inspection Report (if you have access to Edwards, call the Legal Office to set up an appointment for a physical inspection) 8. Insurance Declaration Letter.
