New regulation requires disclosure of registered sex offenders in base housing Published Jan. 12, 2011 By Diane Betzler Staff Writer EDWARDS AIR FORCE BASE, Calif. -- There's a new mandate for base housing residents. Effective this year, changes to Air Force Instructions require installation commanders throughout the Air Force to collect data regarding individuals who are required to register as a sex offender. Current residents and anyone applying for military, government-managed or privatized housing must submit a completed Air Force Form 4422 - Sex Offender Disclosure and Acknowledgement - or documentation as outlined on the form for consideration by the 95th Air Base Wing commander. The form certifies that neither the applicant, nor anyone living with the applicant is registered as a sex offender or is required to register as a sex offender on any national or state sex offender registry. "If the applicant or a family member living with the applicant is registered on any sex offender list, the Air Force requires full disclosure of that information when applying for base housing," said Melody Couch, Edwards' housing manager. Ms. Couch said the requirement for the housing office to collect information on anyone in government housing is a way of protecting all base residents. Those currently living in family housing at Edwards will receive information along with the AF Form 4422 by mail or email. Packages for dorm occupants will be mailed or hand delivered. Residents are required to complete and return the disclosure form or documentation for consideration to the Housing Management Office or the Consolidated Dormitory Management Office depending on whether residents live in a house or in a dorm. The form must be submitted to the HMO or the CDMO by current base residents no later than Jan. 28. "All information submitted on AF Form 4422 is confidential and is protected under the Privacy Act Authority, 5 USC 5911 and 5912. The information is needed to identify housing requirements," Ms. Couch said. If you are a registered sex offender and wish to live on base, or if you are applying for base housing and have a registered sex offender living with you, the information outlined in AF Form 4422 must be submitted along with the application for base housing. Disclosure is voluntary; however, failure to provide the requested information will result in being denied base housing. "Disclosure doesn't necessarily mean base housing will be denied," Ms. Couch said. She said the Air Base Wing commander will review the facts on a case-by-case basis and determine if the applicant or family member is a threat to base population and will take the necessary appropriate lawful action based on the results of the disclosure.
