Package - with GFX - Hawaii National Guard Assists the Department of Health with COVID-19 Mapping

About 30 Hawaii National Guardsmen who were activated to support the State of Hawaiiʻs COVID-19 pandemic response have been assigned to assist the State Department of Health (DOH) in mapping the active COVID-19 cases in Hawaii.

The Hawaii National Guard Soldiers and Airmen have the flexibility to meet needs as they arise, adjusting their skillsets as they face each new request and, have received training the Hawaii Department of Health on Hawaiiʻs state law requirements for handling of personally identifying information.

The Hawaii Department of Health is the lead agency for this effort.

The Hawaii National Guard is providing limited-scope assistance to the Hawaii Department of Heath by way of processing the COVID-19 related data that was given voluntarily by the public.

The material acquired is for the sole use of that Hawaii Department of Health and its investigators who are attempting to mitigate the spread of COVID-19 infections in Hawaii.

For Digital Vaccine Record Request:

If you got your COVID-19 vaccine through the state, public health department or pharmacy, you may be able to request a Digital Vaccine record through the California Immunizations Registry (CAIR) by visiting https://myvaccinerecord.cdph.ca.gov/

If you got your COVID-19 vaccine through the 412th MDG/Edwards AFB, you may request your record be sent to CAIR in order to obtain a Digital Vaccine Record, using the following steps:

1. Log into MHS Genesis Patient Portal (use DS Login or CAC card): https://patientportal.mhsgenesis.health.mil/

2. Send a message to “USAF EDWARDS PHARMACY COMMUNICATION” with phone number and/or email to associate with your vaccine record request.

3. Wait for a reply to your message before requesting through CAIR site (above). 

**Note – it is taking a few weeks to process requests due to overwhelming numbers and backlog. This is a manual data entry push, so please be patient.
